 - Save as 'yourname' to your hard drive
 - Email as an attachment to racemeister@cityleague.com
 - Let us know how you'd like to pay, check or credit card via secure invoicing from PayPal

Note: Credit cards are accepted via email and PayPal. Its easy, secure and fast and you need not be a member.

THIS FORM MUST BE FILLED OUT THOROUGHLY BY ALL RACERS PAST AND PRESENT

Print, fill out, sign, scan, save as a pdf (or jpg) and send attached to an email to racemeister@cityleague.com
